Please find the athletics physical forms that need to be filled out by a healthcare professional HERE.   The healthcare professional will need to sign by the the green highlighted sections and the student and parent will need to sign by the yellow highlighted sections.  

For students to be eligible, they need to have a completed physical packet that is dated on or after May 1st, 2020.
